Tequila customarily comes with a non-poisonous, dead scorpion in the bottle. They're dropped in live and drown.
But as for the movie, Nick drinks from the bottle repeatedly so that when he is lured into the cavern or cove, it dawns on him that the reason the scorpion is dead in the bottle is because it had NO escape. He even says this aloud to himself. Nick ALWAYS made a point of looking for an escape route when he was on a plane or elsewhere. Remember him talking about this earlier in the film? The script writer must have seized the opportunity to use the scorpion in the bottle to parallel Nick being equally trapped in the cavern. The bottle of tequila was not poisoned. The scorpion was there from the start.
